Abstract

Stevens-Johnson syndrome (SJS) or Toxic epidermal necrolysis (TEN) which were previously called Lyell syndrome is the modified form of same skin conditions. Stevens-Johnson syndrome affects less than 10% of total body surface area and can affect anyone with the genetic predisposition of any gender, age or race. According to recent studies, it is more likely to infect 1 of 1000 people with human immunodeficiency syndrome (HIV). What is Stevens-Johnson syndrome?

Stevens-Johnson syndrome (SJS) which was described in 1922 is a rare, cutaneous drug reaction that involves skin and mucous membranes and is characterized by erosions, erythema, blisters, conjunctivitis, purpuric macules, etc. The prevalence of this disease is about 10-30% globally with an incidence of affecting 1-2 individuals per million annually and 1-2 per 1000 with HIV. What are the symptoms of Stevens-Johnson syndrome?

The onset of the disease can have the following clinical features-

• Fever along with severe body aches and cough
• Stinging eyes
• Problem in swallowing
• Erythema 
• Eyelid edema
• Corneal erosion
• Conjunctival sac
• Corneal ulceration
• Hypopigmentation and hyperpigmentation
• Nail dystrophy
• Peeling skin
• Red rashes
• Blisters on the mucous membrane of mouth, throat, eyes, and genitals
• Drooling
• Painful urination
• Large painful erosions

What causes Stevens-Johnson syndrome?

Some of the factors that trigger the syndrome are as follows-  

Medications

Certain medications like acetaminophen (Tylenol), Aleve, Ibuprofen (Advil, Motrin), Antibiotics (Bactrim and Sulphonamides, etc.), Allopurinol (Zyloprim) and medicines that manage mental illness. Drugs like antiepileptic (Carbamazepine, phenytoin, etc.) and Oxicam, etc. are typically associated with Stevens-Johnson syndrome (SJS). Vaccinations for various diseases like dengue, mycoplasma, and cytomegalovirus can be one of the factors that trigger the initiation of the syndrome. (3)

Infections

Some infections like mycoplasma pneumonia can be the reason for the onset of the disease. It can also affect the person with HIV cancer or weak immune system, mumps, flu, cold sores, Bornholm disease, glandular fever, etc.

Family history

Mutation in the human leukocyte antigen (HLA) gene, drug hypersensitivity or certain genetic factors are closely associated with triggering factors.

How can Stevens-Johnson syndrome be to get diagnosed?

Other than the physical noticeable symptoms some of the clinical tests for diagnosing Stevens-Johnson includes the following details-

• Blood tests like Complete blood count (CBC) and Liver function tests (LFT)
• Skin biopsy
